For example, if a patient is dealing with multiple conditions such as hypertension, diabetes, coronary heart disease, liver disease, lung disease, or osteoporosis, they can visit the General Practice Department.
Chronic disease management. The practice integrates disease prevention, treatment, nursing, rehabilitation, and education. It offers comprehensive services including co-management of chronic diseases, health consultations, health assessments, and preventive medical care. For instance, the full course management of chronic diseases such as hypertension, diabetes, and chronic obstructive pulmonary disease.
